How Your To-Do List Is Holding Your Business Back

The Problem With To-Do Lists

A to-do list is infinite. There’s no cap on how many items you can add, no built-in prioritization, and no time commitment attached to any task. So what happens? You scan the list, pick whatever you feel like doing in the moment, and avoid the things that feel hard or uncomfortable, which are usually the things that actually grow your business.

As Lisa puts it, things end up on your to-do list because you don’t want to forget them. But the problem is that we never assign a specific time to actually do the thing on the list. So you’re always choosing based on mood, energy, or whatever feels urgent, not what’s strategic.

Even worse, tasks like “record weekly video” sound simple on a list, but actually involve multiple steps: research, outlining, creating slides, recording, editing, writing the description, and sending the email. When all of that lives as one vague line item, it’s no wonder it keeps getting pushed to tomorrow.

The Fix: Time Blocking On A Calendar

The shift is beautifully simple. Instead of writing tasks on a list, you assign them specific blocks of time on your calendar.

Here’s why this works. A calendar is a fixed container. When you block 90 minutes for recording your weekly video, that’s the time you have. You’ll use it. And when someone calls and asks you to lunch during that block, you treat it the same way you’d treat a patient appointment: “Sorry, I’m working. Let’s reschedule.”

This approach also trains you to understand how long tasks actually take. You might start by blocking two hours for video recording and discover you really only need 90 minutes. Over time, your estimates get sharper, your work gets faster, and you free up capacity for new priorities or rest.

The comparison is striking. Where a to-do list feels overwhelming and infinite, a time-blocked calendar feels organized and realistic. Where a to-do list lets you drift toward whatever feels easy, a calendar enforces specificity and commitment. The decision about what to work on is already made; you just show up and do it.

The Missing Piece: Quarterly Strategic Planning

Time blocking alone is powerful, but without strategic direction, you’ll end up with a beautifully organized calendar full of tasks that don’t matter. That’s just a more organized version of the same problem.

That’s why the second half of this system is quarterly strategic planning. Before you plan your week or month, you need clarity on what you’re working toward this quarter, not just what you’re working on. What are the two or three projects that will actually increase revenue, grow your patient base, or build your audience? Because not every idea is a good idea, and not every task deserves a time block.
